Health Care Services

Cass Family Medicine provides care for all ages – infants, children, teens, adults and seniors.

  • Obstetrical, pediatrics, geriatrics and general medical care
  • Women’s’ health, reproductive health, prenatal, obstetrics and gynecology
  • Pharmacy
  • Blood testing for lead
  • STD testing and treatment
  • Immunizations
  • Cardiovascular, diabetes, cholesterol and depression screenings and treatment

Pap smears, school physicals, prenatal visits, well baby exams, physicals and other routine healthcare procedures are a large percentage of the clinic’s everyday appointments. Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns and other on-going health problems are also routine activities at the clinic.

Behavioral Health?

Behavioral Health and Social Work providers work collaboratively with the medical providers to help patients with a variety of issues affecting their overall health. When a patient sees a medical provider and psychosocial or behavioral issues are identified, a behavioral health staff member or social worker will be called in to consult with the patient and/or family during the visit. Follow up appointments are made when necessary.??Behavioral health providers assist patients in dealing with problems such as depression, anxiety, pain management, ADHD, lifestyle changes related to chronic diseases, children’s behavioral problems, parenting concerns, family violence, smoking cessation, chemical dependency, stress management, and marital and family conflicts. OneWorld social workers assist patients with accessing social support services, confronting financial and housing problems, violence and sexual assault, literacy, and many other issues.

Health Education?

Health education is integrated into each patient’s visit. Nutrition, disease prevention and management are taught to patients individually and in groups.

Social Services and Disease Management?

Cass Family Medicine offers educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care and care for chronic diseases: asthma, diabetes, hypertension, high cholesterol and depression.

Medicaid Enrollment?

Cass Family Medicine provides assistance in applying for Medicaid. To qualify for Medicaid, a person must be a citizen or legal resident of the United States and meet income guidelines. Unborn children are also eligible for Medicaid.

Specialist Referrals?

Referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.
