Cass Family Medicine accepts cash, check, Visa, MasterCard, or Discover, most forms of insurance, Medicare, and Medicaid.
For people without insurance we base our charges on a sliding fee scale. Proof of one month's worth of income is required to determine the fees for services rendered.

The cost above is only an estimate based on our guidelines. These rates are subject to change. Referral services follow a different sliding fee scale. This is also affordable when compared to competitive prices, which are not reflected in the above scale. Cost for Dental Services vary. This is also only an estimate. When you schedule an appointment you will meet with a financial counselor who will go over payment with you at that time. Cass Family Medicine is committed to serving both insured and uninsured patients. We also accept those on Medicare or Medicaid. For those with limited income and who do not have insurance, the sliding fee scale is available. Charges are based on family size and income level.
Medicaid Enrollment
Cass Family Medicine accepts all forms of Nebraska State Medicaid. We also offer assistance in applying for Medicaid. To qualify for Medicaid, a person must be a citizen or legal resident of the United States and must meet income guidelines. Medicare
Cass Family Medicine accepts most Medicare plans and supplementary insurance.
Commercial Insurance Providers
Cass Family Medicine accepts and welcomes most forms of commercial insurance, including Offutt Air Force Base’s Tricare. Please bring your insurance information with you to your appointment.
